Join our dynamic healthcare team at Delight Care as a Home Health Registered Nurse, where you will play a vital role in delivering compassionate, comprehensive care to patients in their homes. This position offers the opportunity to utilize your critical care, Inpatient, and outpatient experience to improve patient outcomes. As a key member of our interdisciplinary team, you will assess patient needs, develop personalized care plans, and coordinate services to ensure safe and effective recovery. Your expertise in acute, and post-acute care will be essential in managing diverse cases across various settings. This role is perfect for motivated nurses eager to make a meaningful difference in patients’ lives while working in a flexible, supportive environment.

Responsibilities

• Conduct thorough patient assessments including vital signs, physiology evaluation, and medical history review to determine individualized care needs.
• Develop and implement comprehensive care plans aligned with best practices in home health, hospice, and post-acute care.
• Manage patient discharge planning and coordinate with healthcare professionals to ensure seamless transitions from hospital or clinic settings to home care.
• Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, clinical notes, and treatment updates while maintaining HIPAA compliance.
• Perform diagnostic evaluations and oversee medical-surgical interventions as needed within the home environment.
• Collaborate closely with physicians, therapists, social workers, and other healthcare providers to optimize patient outcomes through case management and utilization management strategies.
• Educate patients and their families on health conditions, medication management, infection control practices—including infectious disease care—and self-care techniques.

Qualifications

• Valid RN license with current registration; additional certifications in critical care or ICU preferred.
• Proven experience in hospital medicine, emergency medicine, ICU, or Level I/II trauma centers; home health or hospice experience highly desirable.
• Strong knowledge of medical terminology, physiology, health regulation policies, and HIPAA standards.
• Demonstrated ability to perform comprehensive patient assessments and manage complex clinical cases effectively.
• Proficiency with EMR/EHR systems such as Kinnser Wellsky for documentation and electronic health records management.
• Excellent communication skills for client interaction and interdisciplinary collaboration.
• Ability to adapt quickly to diverse clinical scenarios including behavioral health, geriatrics, pediatrics (PICU), outpatient clinics, and managed care environments.
• Wound Care and IV experience are desired.
