Responsibilities:

• Provide travel physical therapy services in a home health setting.
• Use HCHB and OASIS for the required home health workflow.

Qualifications:

• HCHB experience.
• OASIS experience.

Schedule:

• Monday through Friday.
• 36 to 40 guaranteed hours per week.
