Description

A Community Health Worker (CHW) at the John Reed Family Resource Center (FRC) serves as a trusted bridge between individuals, families, and health and social service systems. The CHW's primary role is to reduce barriers to care for marginalized and underserved communities by providing culturally responsive outreach, health education, and resource navigation. Under the supervision of the Program Manager, the CHW supports families at the FRC by promoting health and improving access to essential resources.

• Fluent Spanish/English speaking and writing.
• High school diploma or equivalent.
• Hold a valid California driver's license along with reliable transportation.
• Basic computer skills, including familiarity with Microsoft Office suite, Google suite, online research and video conferencing apps.

ESSENTIAL FUNCTIONS:

• Learns the various community resources in order to connect and/or refer community residents with health care and related services.
• Connect families to essential service, including not limited to: food assistance, housing support, legal resources, and energy or utility aid based on identified needs.
• Provide culturally responsive education on health management, nutrition, and preventive care to promote informed healthy lifestyle decisions.
• Assist families in accessing child development resources and school-related needs. Advocate during appointments to help ensure family concerns and needs are clearly communicated.
• Conduct intake assessments, support completion of services and provide ongoing follow-up through phone calls or home visits to maintain continuity of care.
• Participate in community events, facilitate skill-building workshops, conduct presentations, support health awareness, and share Know Your Rights information.
• Prepare, tracks and maintains client case files, logs, and service records in CPI's database.
• Conducts surveys to determine community needs.
• May act as an interpreter and translator when qualified to do for Spanish speaking clients and community residents.
• Attends staff and in-service meetings, approved training programs, and job related educational courses. May participate in special health projects and initiatives.

PHYSICAL REQUIREMENTS:

• Ability to lift, load, bend, and haul relevant items to meetings/events
• Prolonged sitting and working on a computer and telephone.
• Able to stand for extended periods of time.
• Active participation in a virtual training course.
• Must be able to travel to office and community events when needed.
