Description
  • Conduct assessments of patient needs in inpatient and outpatient environments and develop customized treatment strategies

  • Engage with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to improve care outcomes
  • Provide education to patients and families regarding therapeutic objectives, strategies for discharge, and home exercises
  • Maintain comprehensive documentation of assessments, treatment plans, and patient progress according to established standards
  • Take part in care coordination meetings to facilitate integrated patient care
Education
  • Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized institution
Certifications
  • State licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (e.g., focus on neurology or geriatrics) are beneficial but not mandatory
Skills
  • Prior experience is highly preferred
