Responsibilities

• Assess residents’ physical therapy needs and develop individualized plans of care in collaboration with the care team.
• Provide direct therapy, including therapeutic exercise, gait training, balance work, mobility training, strength, and functional activities.
• Implement evidence-based treatment techniques and modalities appropriate for older adults in a SNF setting.
• Reassess progress, document progress notes, and adjust plans of care to meet patient goals.
• Collaborate with physicians, nursing staff, and therapy assistants; communicate changes in patient status.
• Ensure timely documentation and adherence to facility protocols and regulatory requirements.

Requirements

• Active physical therapy license in New Hampshire (or eligibility to practice per state rules)
• Availability for a 13-week travel assignment in this location
• Experience in skilled nursing facility or geriatric rehabilitation settings preferred
• Strong communication and teamwork skills
• Ability to complete required documentation and comply with regulatory standards
