This provider will collaborate with physicians, nurses, facility staff, and other healthcare professionals to develop individualized care plans and drive optimal healing outcomes. The ideal candidate brings strong clinical judgment, comfort working independently in the field, and a patient-centered approach to care.ResponsibilitiesPerform comprehensive wound assessments, including wound dimensions, characteristics, tissue type, signs of infection, and pain levelsDevelop and implement individualized wound care treatment plans based on patient condition, history, and clinical needsProvide advanced wound care interventions, including wound cleansing, debridement, dressing application, and wound closure techniques as appropriatePrescribe appropriate medications, including topical agents, antibiotics, and pain management medications, as neededOrder and interpret diagnostic testing, including wound cultures, laboratory studies, and imaging, to support treatment decisions and healing progressEducate patients, families, caregivers, and facility staff on wound care techniques, prevention strategies, and treatment plan adherenceMonitor wound healing progress, document findings thoroughly, and adjust treatment plans as neededCollaborate with physicians, surgeons, vascular specialists, therapists, and other care providers to ensure coordinated treatment for complex wound casesMaintain timely, accurate, and complete documentation in the electronic medical recordStay current on wound care best practices, technologies, and evidence-based treatment approachesParticipate in quality improvement initiatives, audits, and other wound care-related clinical effortsQualifications2+ years Nurse Practitioner experienceActive and unrestricted Nurse Practitioner license in the state of practiceBoard certification as a Nurse Practitioner requiredCurrent DEA license and prescriptive authority, or ability to obtain upon hireWound care experience strongly preferredExperience in post-acute care, skilled nursing, long-term care, rehab, or related settings preferredStrong clinical assessment, diagnostic, and treatment planning skillsExperience performing wound care procedures such as debridement, dressing selection, and ongoing wound management preferredAbility to work independently while managing a mobile, field-based scheduleStrong communication and relationship-building skills with patients, families, and facility partnersComfortable using EMR systems, tablets, and mobile technology in daily practiceValid driver's license, reliable transportation, and ability to travel locally between assigned facilitiesOrganized, dependable, and adaptable, with a high level of professionalism and accountabilityStrong personality and cultural fit for a collaborative, fast-growing organizationPreferredEnjoys working in a non-hospital settingValues autonomy and efficiencyBrings a compassionate, patient-centered bedside mannerCan build rapport quickly with facility staff and patientsThrives in a growth-oriented organization with strong team cultureInterview ProcessOne phone interviewOne virtual Teams/Zoom interviewNo in-person interview required prior to hiring decision
