Primary Care Physician

Overview We are seeking a Primary Care Physician (PCP) to provide comprehensive, outcome-driven care for a panel of up to 450 patients. The role involves diagnosing, treating, managing, and coordinating care for patients with acute and chronic conditions, especially older adults. The PCP acts as a health coach, helping patients set and achieve health goals, and collaborates with care teams, specialists, hospitalists, and transitional care teams to ensure continuity of care. Responsibilities Lead care teams and ensure compliance with quality standards. Diagnose, treat, manage, and coordinate care for patients with acute and chronic conditions (with emphasis on older adults). Promote a positive center culture and use evidence-based practice in all care decisions. Collaborate with care teams, specialists, hospitalists, and transitional care teams to ensure continuity of care. Utilize digital tools to support patient care and population health. Qualifications Strong leadership, evidence-based practice, and excellent communication skills. Commitment to quality care and patient-centered outcomes. Proficiency in digital tools and willingness to travel locally and nationally. Experience caring for older adult populations is preferred. #J-18808-Ljbffr
