Description

• Conduct assessments for patients in both inpatient and outpatient environments and develop tailored treatment plans
• Implement physical therapy strategies aimed at enhancing mobility, strength, balance, and overall function
• Engage with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and prescribed home exercises
• Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with regulatory standards
• Participate actively in multidisciplinary team rounds and conferences to provide well-coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited program

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (such as neurological or geriatric) are advantageous but not compulsory

Skills

• Prior experience considered highly valuable
• Adept at treating patients with multifaceted medical and mobility challenges
• Excellent communication abilities and team collaboration skills
