About the Role
We are seeking a compassionate Hospice Registered Nurse (RN) to provide patient-centered hospice care in community settings. The Primary RN will manage an assigned caseload, coordinate interdisciplinary care, and support patients and families with symptom management, education, and end-of-life care.

About the Role
We are seeking a compassionate Hospice Registered Nurse (RN) to provide patient-centered hospice care in community settings. The Primary RN will manage an assigned caseload, coordinate interdisciplinary care, and support patients and families with symptom management, education, and end-of-life care.
Responsibilities
- Manage an assigned caseload of hospice patients in home and community settings.
- Assess patient conditions and develop individualized plans of care.
- Provide symptom management and compassionate end-of-life nursing care.
- Educate patients, families, and caregivers on disease progression and care needs.
- Coordinate care with physicians and the interdisciplinary hospice team.
- Maintain accurate documentation and participate in weekend/on-call coverage as needed.
- Active Registered Nurse (RN) License .
- Minimum 2 years of RN experience .
- Hospice nursing or case management experience preferred.
- Strong assessment, critical thinking, communication, and patient education skills.
- Ability to work independently in home and community care settings.
- Willingness to provide occasional weekend backup and on-call coverage.
