Education Requirements

• MD or DO degree from an accredited institution.
• Completion of an accredited residency in Family Medicine.

Certifications

• Board Certified in Family Medicine.
• Eligible for or currently possess an active medical license in Texas.
• BLS and ACLS certifications are desirable.
