RN Field Care Manager

Humana

Humana is seeking an RN Field Care Manager to deliver personalized, community-based care management for our members. In this field role, you will conduct in-home assessments, develop and coordinate individualized care plans, manage chronic conditions, and address social determinants of health. You’ll collaborate with physicians, interdisciplinary teams, and community partners to improve outcomes, reduce avoidable hospitalizations, and enhance the member experience. This position combines autonomy in the field with strong support, development opportunities, and comprehensive benefits in a mission-driven, member-focused culture.
