Overview The ACCS Clinical Program Director promotes successful community integration and the overall well‑being of adults with psychiatric disabilities and co‑occurring Substance Misuse or complex medical needs. Working in an Intensive Medical Group Living Environment, the ACCS Clinical Program Director will oversee the clinical, financial and administrative operation of a Intensive Medical Group Living Environment site to ensure that each Person Served receives the best the services needed to live successfully independently. The Clinical Program Director will supervise staff, ensure quality of services licensing, quality, and oversee the creation and implementation of treatment plans. The ACCS Clinical Director will also facilitate successful and timely transitions to and from residential settings. Responsibilities The essential job duties/responsibilities of the position include but are not limited to the information listed below: Promote the engagement, housing permanency, community tenure, achievement of greater self‑sufficiency and successful completion of tenure for all PS living at the site. Oversee Treatment Plans developed by IM GLE Nurse, and the service delivery to all assigned PS, including participation in services, treatment and support skills planning and implementation of recovery goals, peer and family support, readiness for discharge, and support through service and life transitions, including transitions to and from acute care settings as needed. Ensure that management of the complex medical conditions of PS are continuously addressed, including diabetes; kidney, pulmonary, mobility, physical safety and end of life issues; medical consequences of HIV/AIDs virus, and incontinence, etc. Ensure staff’s hands‑on assistance to PS for grooming, showering, dressing, etc. With IM GLE Nurse, coordinate support services, including hospice and the involvement of religious and spiritual supports for PS and their families/other supporters. Create and maintain meaningful day activities that address PS Treatment Plan goals. Assist IM GLE Nurse in selecting interventions strategies for residents’ Treatment Plans. Oversee the quality and timeliness of all staffs’ Treatment Plan documentation and Medicaid Rehab Optionstandards adherence. Obtain consultation from ACCS Consulting Occupational Therapist, ACCS Consulting LPHA Clinician, Vinfen Director of Nursing, Medical Director, Assistant Medical Director as needed to address clinical and Treatment Plan needs of PS. Develop and train staff to implement residents’ Safety Plans and Crisis Prevention and Response Plans. With IM GLE Nurse, maintain the residence’s adherence to Medication Administration Program policies and procedures. With IM GLE Nurse, serve as liaison to and maintain medical care access and responsiveness, and strong working alliances with mental health, medical and Care Coordination entities. Manage the site’s systems and procedures, including planning and implementing regulations and requirements, quality improvement initiatives, and intakes and discharges. Create a helping environment in the residence in which family, friends, and others important in the lives of PS are welcome, respected and valued. Promote healthcare and employment services access for PS. Perform, train, supervise, mentor, model evidenced‑based clinical and recovery oriented interventions for PS, including Motivational Interviewing; Harm Reduction and Addiction Treatment support; Screening; Brief Intervention, Referral and Treatment (SBIRT); Housing First; Stages of Change; Wellness Recovery Action Plans (WRAP); Seeking Safety and Cognitive Behavioral interventions. Provide competency‑based training for all staff to achieve successful treatment outcomes for PS. Mentor and model the use of best practices and evidence‑based practice interventions for all involved staff per PS Treatment Plan specifications. Promote teamwork between all staff to ensure all Treatment Plan interventions in and out of residence are implemented and completed. Manage adverse events, including Incident Reporting and ensuring corrective actions are implemented. Monitor the staffs’ use of evidence‑based practice techniques, request additional training from consultants as needed. Manage the site’s DMH licensing adherence. Manage residential sites’ collection and use of resources including food stamps, program fees, representative payee accounts and Client Funds. Monitor expenses and residence’s budget. Submit information for billing procedures. Oversee Human Rights policies and procedures for residence. Ensure the maintenance, repair and safety of the residence. Maintain “good neighbor” relationships with residence’s neighbors. Hire, orient, supervise and evaluate residence’s staff. Offer consistent and direct performance feedback. Conduct corrective action procedures for residential staff if needed. Manage relief and overtime resources to maintain operational excellence. Implement retention, recognition and awards initiatives for residence staff; provide career development guidance and broadcast promotion opportunities. Ensure required group residential Performance Measures are maintained. Train and support staff training in, and use of trauma informed, recovery‑oriented, person‑centered, culturally competent and strengths‑based evidence‑based practice approaches and skills training techniques in their work. Provide thorough and thoughtful documentation of PS needs and services. Promote employment and education services access for PS with MRC and Clubhouses as applicable to the needs of PS in the residence. Participate as member of Intensive GLE Program Director Community of Practice Workgroup led by the Clinical Directors for continuous professional role competency development, licensure specific training, mentoring and learning. Receive needed professional Licensure supervision by Clinical Directors (or Director of Nursing if RN). Collaborate as needed with ACCS team staff. Provide on‑call coverage as assigned. Perform other related duties, as assigned.
