Licensed Social Worker – Case Manager | Case Management (BHS)
Gateway Regional Medical Center - Granite City, IL
Full-Time | M–F, 40 hrs/wk | $25.71–$41.65/hr

Gateway Regional Medical Center is looking for a Licensed Social Worker to join our Case Management team. You'll work independently to review medical records, assess medical necessity, and help guide appropriate hospital stays and ancillary services - all in alignment with Joint Commission and Medicare standards. It's a role that blends clinical judgment with genuine advocacy for patients navigating their care.

This is a great fit for a social worker who wants meaningful, independent work and the satisfaction of directly shaping patient outcomes within a collaborative case management team.

What You'll Do

• Provide social services in accordance with TJC and Medicare standards
• Conduct daily review of medical records to assess appropriateness and medical necessity of admission, continued stay, and ancillary services
• Function independently within the direction set by Case Management
• Support patients and families through care transitions and discharge planning

What You Bring

• MSW from a CSWE-accredited program, or a BSW from a CSWE-accredited institution
• Illinois LSW licensure (required within 6 months of hire)
• LCSW or LMSW preferred
• Prior healthcare experience preferred
