LOCATION: Dublin, GA to Eastman, GA / Float between assigned locations based on staffing needs. 

The Community Service Board of Middle Georgia is seeking a qualified individual to serve as our next LPN. This person will be responsible for m aintaining complete, accurate healthcare records and reports.  Documenting vital signs and psychiatric/medical information. Working with the medical staff to ensure that the health and safety of our clients are being best taken care of.

Responsibilities of the LPN:

• Working with physicians, PA's, NP's via zoom and in clinic.
• Maintaining complete, accurate healthcare records and reports.
• Medication administration, including injections.
• Monitoring patients for side effects or reactions. Phlebotomy.
• Documenting vital signs and psychiatric/medical information.
• Monitoring, recording, and reporting symptoms or change in patients conditions and responses.
• Effectively communicating with patients and their families, providers, and others.
• Complete prior authorizations and patient assistance applications in a timely manner.
• Return phone calls and answer messages.
• Ability to multitask while providing a high standard of care to our patients and be able to work autonomously as a team.
• Maintain HIPAA regulations.

Here are some of the things we require:

• High School Diploma/GED Equivalent
• Valid Georgia Driver’s License
• Effective verbal and written communication skills
• Strong interpersonal skills and the ability to work effectively with diverse communities
• Ability to work independently and in collaboration with others
• Experience with Microsoft 365 Office Products
• Ability to organize, prioritize and meet deadlines accordingly
• LPN must be licensed to practice in the state of Georgia,
• Must possess basic computer and typing skills,
• Ability to work in Adult Outpatient
