As our Care Manager RN, you will be a dynamic leader, shaping clinical excellence and empowering your frontline nursing team to deliver unparalleled patient care. • Conducts in-depth assessments/reassessments covering medical, health, and rehabilitation concerns. Performs physical assessments as necessary and interpret clinical findings
• Certifies level of care determinations
• Works closely with physicians and other health care professionals. Consults with team members and participates in team care management activities
• Develops care plans, implements and monitors services, consults with other team members, completes case recording and reporting. Maintains case load
• Participates with CCT providing assessments, feedback and planning for patients
• Assists with the development of organizational programs to create new services

Required:

• Three (3) years experience two (2) of which are in public health
• Associate of Science degree in nursing
• Valid current California license to practice nursing

Preferred:

• Geriatrics experience
• Bachelors degree in nursing
