Role Description This full-time, on-site Physical Therapist role is based in Indianapolis, IN. The Physical Therapist will assess patient conditions, develop individualized treatment plans, and deliver evidence-based therapeutic interventions to improve mobility, function, and quality of life. Daily responsibilities include coordinating care with nurses and other healthcare professionals, educating patients and caregivers on exercise programs and safety, and documenting clinical assessments, progress notes, and outcomes in accordance with regulatory and company standards. The role may involve providing services in home care or facility-based settings, ensuring continuity of care and adherence to Interim HealthCare’s quality and patient-centered care principles. The Physical Therapist will also participate in interdisciplinary meetings, contribute to discharge planning, and support ongoing rehabilitation goals. Qualifications Strong foundation in Medicine and Physical Therapy, with the ability to apply clinical knowledge to diverse patient populations. Demonstrated expertise in Rehabilitation, including designing and implementing individualized therapeutic exercise and mobility programs. Proven skills in Patient Care, with a focus on safety, functional improvement, and person-centered communication. Experience or interest in Home Care, including working in patients’ residences and adapting treatment plans to home environments. Current state licensure as a Physical Therapist in Indiana (or eligibility to obtain), and graduation from an accredited Physical Therapy program. Excellent written and verbal communication skills, including accurate clinical documentation and collaboration with interdisciplinary teams. Ability to work independently, manage a varied caseload, and prioritize tasks in compliance with regulatory and company standards. CPR certification and familiarity with infection control, safety protocols, and ethical standards in patient care. Previous experience in home health, hospice, or senior care settings is preferred but not required. #J-18808-Ljbffr
