Requirements

‍⚕️ Experience Requirements

Required:

• 2 years of acute-care case management/discharge planning
• Travel experience required, although first-time travelers may be screened
• CA RN license required by start date
• Pending CA license accepted if obtained before start
• Epic/charting experience required

Highly preferred:

• InterQual experience
• Admission review experience
• Care coordination
• Discharge planning
• Medicare guidelines
• Medical necessity determination
• Acute hospital experience

Main Responsibilities

The RN Case Manager will be expected to independently handle:

• Patient assessments
• Care coordination
• Admission reviews
• Discharge planning
• Medical-necessity reviews
• InterQual criteria
• Family meetings
• Treatment-plan development
• DME arrangements
• SNF placement
• Ambulance/transport arrangements
• Coordination with HMOs, IPAs, Medicare, Medi-Cal, etc.
• Weekend coverage, including potentially monitoring up to 2 units and focusing on discharge planning

Patient/Caseload

• Day ratio: Approximately 1:20
• Case load: Up to 25 patients
• Patient population: Neonates through geriatrics
• Weekend: Every other weekend

Pay/Billing Details

• $2100 - $2350 Gross Weekly
