Home-Based Care Plan Coordinator - Medicaid Waiver

The Information Center, Inc. in Michigan is seeking a dedicated RN/SW team member to assess medical, functional, psychological, financial, and environmental needs of MI Choice Medicaid Waiver participants, developing person-centered plans with participants and families. You will conduct in-home assessments, coordinate with physicians and community resources, monitor progress, and ensure documentation and reassessment every three months, while upholding professional ethics and agency standards. #J-18808-Ljbffr The Information Center, Inc.
