Case Manager
Three Rivers Treatment Center is seeking a compassionate, organized, and highly collaborative Case Manager to join our clinical team. Under the general supervision of the Clinical Director, the Case Manager is responsible for coordinating services and supporting the treatment needs of adolescent residents in a residential behavioral health setting. The Case Manager serves as a key liaison between residents, families, referral sources, physicians, treatment team members, community providers, insurance/reimbursement representatives, schools, social service agencies, courts, and other community partners. This position plays an essential role in care coordination, treatment planning, family engagement, community collaboration, and discharge planning.
The ideal candidate will have strong communication skills, an understanding of behavioral health services, and the ability to build effective relationships with families and community partners.
Key Responsibilities
- Coordinate comprehensive case management services for residents throughout their treatment stay.
- Collaborate with the Clinical Director and multidisciplinary treatment team to develop, implement, monitor, and update individualized plans of care.
- Participate in assessments and evaluations to assist in determining appropriate treatment and placement needs.
- Utilize knowledge of the DSM-5 and behavioral health criteria when supporting screening and placement decisions.
- Coordinate treatment services and communicate resident progress with families, referral sources, providers, and other authorized parties.
- Develop and maintain comprehensive plans of care and ensure appropriate documentation is completed.
- Complete required weekly case management notes and other clinical documentation.
- Coordinate and manage the treatment team schedule.
- Facilitate communication between residents, families, physicians, clinicians, nursing, education, and other members of the treatment team.
- Provide monthly updates to referral sources and families in collaboration with the clinical team.
- Notify families and community partners of changes to treatment team schedules and meetings as appropriate.
- Facilitate family engagement and encourage active participation in the resident's treatment process.
- Assist with identifying appropriate community resources and services to support residents and families.
- Develop and provide recommendations for discharge planning.
- Coordinate with outside providers to support continuity of care following discharge.
- Assist in maintaining an appropriate length of stay based on the resident's clinical needs, treatment progress, and discharge goals.
- Maintain working knowledge of FAPT, CSA, community behavioral health resources, social service agencies, insurance carriers, and other applicable community resources.
- Serve as a liaison between the facility and reimbursement/insurance representatives when appropriate.
- Participate in interdisciplinary and interagency meetings, conferences, seminars, and training activities.
- Maintain accurate, timely, and confidential documentation in accordance with facility policies and regulatory requirements.
- Maintain strict confidentiality of resident and family information in accordance with HIPAA and facility policies.
- Support facility initiatives and provide actionable feedback to improve services and resident outcomes.
- Share best practices and serve as an advocate and information resource for the facility.
- Partner with the Marketing and Community Relations team to identify opportunities to strengthen community partnerships.
- Participate in community outreach, education, and relationship-building efforts throughout Virginia.
- Represent Three Rivers Treatment Center at community events, health fairs, professional meetings, speaking engagements, and other outreach opportunities as assigned.
- Attend required staff development, in-service training, and regulatory training.
- Perform other duties as assigned that are reasonably related to the position.
Community Outreach & Partnerships
The Case Manager will play an important role in developing and maintaining positive relationships with community partners and referral sources. Responsibilities may include:
- Building relationships with hospitals, schools, social service agencies, behavioral health providers, courts, physicians, and other referral partners.
- Educating community partners about available services and treatment programs.
- Supporting referral development and community outreach initiatives.
- Attending community events, health fairs, conferences, and professional networking opportunities.
- Providing feedback to leadership regarding community needs and opportunities to improve services.
Required Qualifications
- Bachelor's degree required.
- Minimum of one (1) year of experience in a behavioral health, mental health, or therapeutic setting serving the population of the facility.
- Strong written and verbal communication skills.
- Ability to work effectively as part of a multidisciplinary treatment team.
- Ability to maintain professional boundaries and confidentiality.
- Must be at least 19 years of age, in accordance with applicable Commonwealth of Virginia requirements for children's residential facilities.
- Strong organizational skills and attention to detail.
- Ability to manage multiple priorities and meet documentation deadlines.
Preferred Experience
- One (1) year of experience in intake and/or patient/resident assessments.
- One (1) year of experience with treatment planning and coordination of care.
- Experience working with adolescents and families in a behavioral health or residential treatment environment.
- Knowledge of Virginia's CSA/FAPT system and community-based behavioral health resources.
- Experience working with insurance companies or reimbursement representatives.
- Experience with discharge planning and community transitions.
Licensure & Certifications
- Current CPR certification.
- Current First Aid certification.
- Completion of the facility's Behavioral Management Program within 14 days of hire.
- Valid driver's license in good standing.
