Street Outreach Case Manager — Housing Navigator
St. Joseph Center
The Case Manager at St. Joseph Center works with a multi-disciplinary homeless street outreach team to ensure quality care in line with LA DHS rules and the Center's mission, helping people experiencing homelessness access resources for permanent housing. Key duties include street outreach, coordinating housing programs, linking clients to health and benefits, entering data in HMIS, and attending trainings and meetings.
