Responsibilities Develop personalized care plans and connect individuals living with developmental disabilities to essential services and supports. Provide advocacy, counseling, crisis intervention, and daily living support to promote independence and inclusion in the community. Support basic physical, financial, social, and personal needs. Monitor progress, facilitate connections with service providers, and empower individuals to lead fulfilling lives. Qualifications Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling. One (1) year of experience providing case management or support coordination services to adults living with a developmental disability. Valid Driver's license for transporting clients as necessary. In addition, success in this role includes having in-depth knowledge of intellectual and developmental disabilities (IDD), autism, and other developmental, physical, emotional, and sensory disorders.
