Location: Morristown, NJ

Duration: 13 Weeks

Schedule Notes: 8a - 4p (Rotating weekends every 4th or 5th) 1 summer/1 winter holiday

Pay Rate: $57/HR ON W2 Must have exp. with Sub-acute pts.

Duties:

• Coordinates patients through the acute care hospitalization
• Manages all aspects of discharge planning/transition process.
• Performs utilization management and quality screenings In-Patient Case Management is required.

Skills:

• 2 years of acute care experience strongly preferred.
• Case Management, Managed Care. In Patient Case Management is required.

Requirements:

• BSN required
• RN license required
• CCMC preferred
