RN Case Manager: Care Coordination & Discharge Planning

AHMC HealthCare seeks a Case Manager Registered Nurse to manage the preparation, recording and tracking of payments for hospital services. The role involves handling calls from insurers, SNFs, home health agencies and other providers, ensuring smooth prioritization for quality patient care. It also coordinates with Discharge Planners for continuum of care, including DME and levels of care. Applicants must hold a CA RN license, type 45–55 wpm, and have strong medical terminology, with proficient #J-18808-Ljbffr
