Role: Case Manager, Social Work – In Patient II Responsibilities Conduct psychosocial assessments of hospitalized members Develop, implement, and monitor care and discharge plans Collaborate with physicians, nurses, and allied health staff to ensure safe care transitions Advocate for patients and families, connecting them with community and behavioral health resources Address and resolve barriers to patient recovery Qualifications & Skills Experience in social work or healthcare case management Strong communication and collaboration skills Ability to develop comprehensive care plans and advocate for patient needs Knowledge of community resources and behavioral health services #J-18808-Ljbffr Health Here
