Key Responsibilities

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Serve a diverse patient population ranging in age from birth to geriatric
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Manage an assigned caseload with the ability to travel to patient locations as directed
• Complete thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Participate in weekend rotation schedules approximately once every six weeks

Required Qualifications

• Active Physical Therapist license in the state of Massachusetts is required
• Minimum of 1 year of experience within a certified home health agency is required
• Minimum of 1 year of case management experience is required
• Please note: Experience in hospice, private care, or working as a case manager outside of a certified home health agency does not satisfy the certified home health care experience requirement
• Valid driver's license and reliable transportation required for travel to patient locations

Preferred Experience / Skills

• Experience working with patients across a broad age range, including pediatric and geriatric populations
• Familiarity with interdisciplinary care coordination in a home health setting
• Proficiency with electronic medical records and documentation platforms
