Hospice On-Call Registered Nurse RN

On-Call Registered Nurse (RN) Location: Roswell, Alpharetta, Milton, Woodstock, Marietta, Acworth, Kennesaw, Canton, Jasper, Ellijay & Calhoun Three Oaks Hospice, along with our sister organizations—Agape Hospice Care, Sage Hospice, Primary and Palliative Care, Elevation Hospice of Colorado, Elevation Hospice, and Primary and Palliative Care of Utah—invites a compassionate professional to join our hospice and palliative care network. In this role you will provide home‑based skilled nursing care, coordinate interdisciplinary teams, and support patients and families while working within a unified mission of best‑in‑class hospice care. Responsibilities

• Administer skilled nursing care to clients of all ages in their homes.
• Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
• Assume responsibility for care coordination and ensure continuity and quality of service.

Qualifications Graduate of an accredited associate or bachelor’s degree in Nursing. Current state RN license and valid driver’s license. At least two years of clinical RN experience; hospice or home health experience preferred. Current CPR certification. Proficient in computer use; experience with Microsoft Office is preferred. Strong interpersonal skills and ability to provide effective support to patients and families.
