ASSESSMENT COORD (RN/LIC THERAPIST) - OUTPATIENT INTAKE

Assessment Coordinator (RN/Licensed Therapist)
The Assessment Coordinator is responsible for assisting in the process of responding to referrals and other requests for services and information. The Access Counselor provides technical assistance to the discharge planning process, under the direction of the Director of Assessment Referral. This position is also responsible for completing applicable portion of Core Measures (HBIPS) Abstraction Tool for all admissions. This is a remote position with work hours from Monday to Friday, 8 a.m. to 5 p.m.
Job duties include documenting and processing inquiry calls in accordance with Intake policies, assessing and stabilizing patients, notifying on-call doctors, obtaining administrative approval for admissions, completing admission paperwork, following up on missed appointments, obtaining pre-certification, completing outcome studies forms, demonstrating guest relations, notifying administration of problems, identifying safety problems, obtaining medication after hours, and ensuring completion of Core Measures Data (HBIPS) Abstraction Tool.

Qualifications include education as a Registered Nurse or Master's Degree in a human relations or related field, two or more years of experience in Intake assessment activities within an acute psychiatric setting, two or more years of experience in a psychiatric hospital setting including customer contact, and licensure to practice in the State of Texas as a mental health professional. Additional requirements include a working knowledge of the DSM-IV and the Mental Status Exam, a working knowledge of the Addiction and Recovery field, and the ability to work flexible schedules, overtime, and on-call as needed.
