WVU Medicine - Hospice RN - PRN

SHC WVUHS Home Care

Responsible for the direct and indirect nursing care of patients in the home. In collaboration with the patient, family, and interdisciplinary team (IDT), the registered nurse case manager develops, implements, and evaluates nursing care according to agency and system policies. Minimum Qualifications Current Registered Nurse license issued by the state in which services will be provided or a current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). Valid driver’s license. CPR certification within 30 days of hire date. Preferred Qualifications One year of Home Care Nursing experience. Two years of prior nursing experience. Core Duties and Responsibilities Performs the functions of the hospice aide. Performs and monitors basic patient care according to policies and procedures. Provides a safe and comfortable environment in the patient’s home. Ensures privacy and demonstrates concern for the dignity of patient in performing all patient care. Completes assignments in a timely manner. Assumes on‑call responsibility per policy and procedures. Communicates patient care problems or potential problems to the Administrative Staff and/or other IDT members as identified/occur to assure continuity and coordination of care. Provides care, based on physical, psychosocial, communication, safety, education level, and related criteria for the infant, pediatric, adolescent, adult, geriatric patient per the established age‑specific education/standard. States and utilizes admission criteria per agency guidelines. Formulates and establishes a plan of care with the IDT based on assessed findings and updates under physician orders. Completes and documents full system assessment, including assessment of level of physical and psychosocial aspects and identifies age‑specific criteria upon admission and each visit thereafter. Understands pathophysiologic process by taking appropriate nursing action in accordance with changes in patient condition. Reports significant observed changes in patient condition to Administrative Staff and/or physician. Documents patient status and care or services furnished in appropriate areas defined by the documentation policy and procedure. Documents hospice aide supervisory visits as per standard. Performs CPR when directed by patient’s appropriate representative’s wishes. Displays safe, efficient, and prudent use and maintenance of D.M.E. and medical supplies and adaptive equipment; notifies admin/doctor if safely issues R/T DME in home. Demonstrates bag technique per policy and procedure. Applies and maintains equipment according to the Policy and Procedure Manual. Utilizes supplies effectively and to ensure cost containment. Maintains integrity of the IV site while performing patient care. Performs treatments/procedures according to specific standards (e.g., vital signs, patient weights, specimen collection, ostomy care, catheterization, wound care, dressing changes, suctioning, gastric tube). Performs and monitors patient teaching according to the policy and procedures. Coordinates care so that medications are administered as ordered by a physician and in compliance with the medication administration. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard. Identifies the stages of death and dying. Completes IDT forms and Medicare LMRPs per regulations; completes medical updates as per Medicare guidelines; completes patient discharge summary upon termination of services. EVENING & WEEKEND POSITIONS: Audit charts (inactive/active); follow‑up phone call from admissions to assess needs and level of pain or other patients as needed; admissions as needed; assist with patient overflow from the previous shifts/visits; attends IDT meetings and monthly staff meetings regularly or reviews minutes. Physical Requirements Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance. Hearing within normal range. Visual acuity within normal range. Exerting 20 to 50 pounds of force occasionally, or 10 to 25 pounds of force frequently, or greater than negligible up to 10 pounds of force constantly to move objects. Must be able to operate motor vehicle with safe practice. Working Environment Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases. Exposure to toxic gases, fumes, and odors. Exposure to high stress and constant interruptions. Exposure to electrical current. Exposure to radiation from x-rays. Exposure to bad road conditions; exposure to wind, rain, snow, sleet, mud, high and low temperatures, animals and insects, and hazardous roadways. The majority of work is performed in the patient’s home and may require extensive travel. Subject to unpredictable situations; may be exposed to adverse or unpleasant home environments. Skills and Abilities Ability to perform concentrated and complex mental activity with frequent involvement in complex and highly‑technical situations. Ability to work successfully under highly‑stressful conditions and adapt to varying workloads and assignments. Ability to make sound, independent judgments based on scientific principles; collaborate with multidisciplinary team members appropriately. Ability to comprehend and perform oral and written instructions and procedures. Strong communication skills, written and verbal. Mathematical skill to enable calculation of drug dosages and infusion stats. Work during weekends, holidays, and all shifts. Demonstrates knowledge of and compliance with safety policies and procedures. IV skills required.
