Overview PT Physical Therapist – Per Diem (1099) Location: Smithtown Center Salary: $50–55 per hour Benefits Competitive rates 1099 contractor status (not W‑2) Positive work environment Qualifications Degree in Physical Therapy Valid NYS Physical Therapist license At least 1 year of skilled nursing facility (SNF) experience Ability to perform patient evaluations, develop goals, and create individualized treatment and discharge plans Strong documentation skills using electronic records systems Capacity to supervise Physical Therapy Assistants and provide onsite clinical oversight Effective communication skills for patient and family education New graduates are welcomed and encouraged to apply Responsibilities Conduct evaluations/screens, establish patient goals, and develop individualized treatment plans Provide quality physical therapy services with evidence‑based interventions Document progress toward identified objectives in the electronic medical record Supervise and provide onsite guidance for Physical Therapy Assistants Develop and implement new program techniques to enhance PT services Educate patients and families regarding care plans and home exercises Manage workflow efficiently, allocating time for treatment delivery, documentation, and scheduling
