Social Worker (LSW/LCSW) - Hospice

Role Overview The Hospice Social Worker conducts psychosocial assessments of patients and families, identifying emotional, social, and environmental needs related to illness and life circumstances. They develop and implement care plans to enhance family support, maximize benefits, and promote dignity and coping skills, collaborating with interdisciplinary teams. Responsibilities include providing short-term counseling, crisis intervention, and resource support such as community services, financial assistance, and advance directives. They address spiritual and cultural considerations, respond to imminent death indicators, and facilitate self-determined care decisions. The role involves thorough documentation within electronic medical records, evaluating intervention effectiveness, and ensuring compliance with hospice policies and Medicare benefits. The worker also assists with barriers to care like transportation, housing, and access to resources, making appropriate referrals and supporting family needs. Qualifications & Skills A Master’s Degree in Social Work, licensure as an LSW or LCSW, and at least one year of healthcare social work experience are required. Valid driver’s license and auto insurance are necessary. The ideal candidate demonstrates cultural sensitivity, strong communication skills, crisis management abilities, and knowledge of hospice care principles, including Medicare benefits.
