Position Details:

• Location: Watertown, CT
• Shift Options:
  • Friday/Saturday: 11:00 PM - 7:00 AM (Choose to work every weekend or every other weekend)
  • Saturday/Sunday: 7:00 AM - 3:00 PM (Choose to work every weekend or every other weekend)

Your Role & Responsibilities:


As an LPN on our team, you will play a vital role in enhancing the lives of individuals through personalized care and support. Your responsibilities include:

• Providing respectful and effective communication with clients, families, and team members
• Assisting with personal care, hygiene, laundry, and grooming
• Administering healthcare treatments following instructions from physicians, RNs, or approved staff
• Educating individuals on personal hygiene, nutrition, and behavioral skills
• Ensuring safety through emergency procedures, proper lifting techniques, and safe transfers
• Supporting nutritional needs by monitoring mealtime assist and serving food appropriately
• Engaging clients in social, recreational, and community activities, including shopping errands
• Transporting individuals safely to appointments and outings
• Collaborating with internal and external partners to address challenges and ensure quality care
• Modeling teamwork, professionalism, and responsibility to support a positive workplace environment

Qualifications:

• Valid LPN license
• Minimum age of 18
• Valid driver's license and reliable transportation
• Prior experience working with individuals with disabilities is a plus, but we welcome applicants eager to enter the direct care field
