We're seeking a dedicated Licensed Practical Nurse (LPN) to join our care team and provide high-quality hospice services to patients in their homes.This is a rewarding opportunity to make a meaningful difference every day by providing hands-on care, education, and emotional support to patients and families.What You'll DoDeliver direct nursing care under the direction of a Registered Nurse (RN) in accordance with the plan of careAdminister medications and treatments as ordered by the physicianMonitor and report changes in the patient's condition and ensure timely communication with the RN and interdisciplinary teamProvide education to patients and caregivers on self-care, medications, and symptom managementAssist with daily living activities to promote patient comfort and independenceMaintain accurate clinical documentation and timely reporting of patient care and progressParticipate in interdisciplinary team meetings, quality improvement initiatives, and continuing educationEnsure safe, respectful, and compassionate care at all timesQualificationsLicensed Practical/Vocational Nurse (LPN/LVN) in the state of employmentGraduate of an accredited Practical or Vocational Nursing programMinimum 1 year of nursing experience (hospice, home health, or related preferred)Active Basic Life Support (BLS/CPR) certificationValid driver's license, auto insurance, and reliable transportationStrong communication and time management skillsComfortable working independently and in rural or home settingsDemonstrates compassion, empathy, and a deep understanding of end-of-life carePlease note: Legacy Hospice is a drug-free workplace.
