Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Engage collaboratively with nursing staff, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and essential home exercises
• Accurately document evaluations, treatments, and patient progression per regulatory and facility guidelines
• Contribute actively in care team discussions and interdisciplinary conferences to guarantee a seamless continuum of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (neurological, geriatric) are advantageous but not mandatory
