Nurse Practitioner at Nest Health

The Nurse Practitioner at Nest Health is designed to improve health outcomes for underserved families through culturally congruent, longitudinal care that integrates physical, behavioral, and social health. Operating within an interdisciplinary team, the NP plays a vital role in advancing Nest's mission to make comprehensive, value-based care radically accessible to America's highest-risk populations.

The Nurse Practitioner (NP) or Physician Assistant is the primary care provider, providing whole-family primary, acute, chronic, and pediatric care to adult and pediatric family members in the mobile/community facility setting, may require some periodic home visits. The NP or PA is both a health practitioner and an educator, delivering care in a culturally congruent, non-judgmental manner and collaborating closely with a clinical team comprised of physicians, behavioral health counselors, registered dieticians, and the partnership of a medical assistant upskilled to help support social services navigation when conducting care in the homes.

Manage a panel of families as their primary care provider, delivering comprehensive longitudinal primary and preventive care, care coordination, and patient education on conditions and medications

Conduct adult and pediatric wellness visits and acute and chronic care requiring physical examination and diagnostic testing in a community facility setting

Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants who support social services navigation

Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals

Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed

Document care and maintain clear, comprehensive care plans addressing patient and family needs

Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance

Participate in community outreach efforts, as needed

Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license

At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred)

Proven skills, knowledge base, and judgment for independent clinical decision-making and problem-solving

Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn

Ability to work full-time, including rotating virtual on-call coverage on occasional evenings and weekends

Current BLS/CPR certification

Passion for serving underserved and vulnerable populations with empathy and respect

Leadership experience in an interdisciplinary care team (preferred)

Startup experience (preferred)

This is a hybrid position that blends travel to rural communities for in-person care with virtual care delivery, offering a dynamic mix of field based and remote service to expand access and continuity of care.

Travel between work locations throughout Show Low and/or Flagstaff, including current and future service areas, may be required in emergent situations based on business needs

Must have a driver's license, a reliable car (mileage is reimbursed), auto insurance, and be willing to follow the Nest's Motor Vehicle Safety Policy.

Must be able to lift to 25 pounds at times (healthcare supplies).
