Pharmacist Inpatient

Join our team as an Inpatient Pharmacist, where you will play a critical role in patient care by ensuring the safe and effective use of medications in a hospital setting. This position is ideal for licensed pharmacists passionate about collaborating with healthcare teams to optimize patient outcomes.

Key Responsibilities:

• Dispense medications accurately based on physician orders and ensure proper dosages for hospitalized patients.
• Monitor patients' medication therapies to prevent adverse reactions and interactions.
• Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust treatment plans.
• Provide guidance on the preparation and administration of intravenous (IV) medications.
• Educate patients and families about medications upon discharge to ensure proper use at home.
• Maintain accurate records of all medications dispensed and ensure compliance with hospital policies and regulations.

Work Environment:

• The role is based in hospital inpatient units, requiring close collaboration with multidisciplinary teams in a fast-paced environment.
