Mental Health Crisis Case Mgr., Emergency Dept- Highland Park Hospital - Night Shift

Position Overview Hourly Pay Range: $35.16 - $54.50. Mental Health Crisis Case Manager – Emergency Department – Night Shift Position located at Highland Park Hospital Emergency Department. Full time, 40 hours per week, night shift. Requires travel between Skokie, Glenbrook, and Highland Park Hospitals. Holidays worked. Responsibilities Conduct crisis clinical assessments for patients seeking treatment and appropriately assess their clinical needs. Obtain initial diagnostic impression and confirm during the disposition process with the physician or designee. Assess patients including vital signs, weight, blood alcohol, and BMI for specific populations. Review previous documentation prior to assessment and develop a case formulation. Provide treatment referrals consistent with the patient’s medical and psychiatric status and symptoms. Identify potential or actual problems that may adversely affect the patient and/or department and report situations immediately. Oversee follow‑up with referral sources as appropriate and obtain collateral with families and support systems through face‑to‑face interviews and/or phone calls. Consult with multidisciplinary treatment team members on assessment, treatment, and discharge planning of patients with mental health problems. 5% Completes and acts as a resource to hospital staff on admission paperwork and patient rights. 5% Contacts and coordinates with families and community agencies as appropriate. 5% Participates in care conferences and emergency department huddles to discuss key social, financial, and discharge planning factors impacting patients’ treatment responses. 5% Facilitates bed management planning and coordination with Endeavour Inpatient Psych units. 5% Educates patients and families regarding the effects of illness and disability on personal adjustment, family dynamics, relationships, and social issues. Follows up on documentation with local and state agencies as a mandated reporter per HIPAA guidelines. Qualifications Master’s degree or higher in Behavioral Health field. Current State of Illinois Registered Nurse or Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT). Experience with group, individual, and family therapy; adolescents and families in crisis. Familiarity with Cognitive Behavioral Therapy and Dialectical Behavioral Therapy. Minimum of two years of experience in crisis/emergency services, behavioral health setting preferred. Knowledge of patient rights and laws pertaining to mental health.
