Value-Based Primary Care NP — Advanced Practice Leader

Hispanic Alliance for Career Enhancement

Oak Street Health is recruiting a Nurse Practitioner to provide value-based primary care in outpatient clinics for adults on Medicare. You will work with a physician-led team of medical assistants, nurses, and care coordinators to keep patients healthy, informed, and connected to their care plans. The role features independent patient visits, wellness exams, chronic disease management, care coordination with specialists, and home visits as needed. #J-18808-Ljbffr Hispanic Alliance for Career Enhancement
