Responsibilities Ensure timely patient progression of infusion patients from hospital to home. Act as the first point of contact for new home health referrals and clinical coordination. Review clinical documentation and referrals to ensure appropriateness and readiness for home infusion services. Coordinate with physicians, hospital discharge planners and the interdisciplinary team to develop individualized plans of care. Serve as the front‑line resource for patient and nursing staff questions. Support quality outcomes and continuity of care through proactive communication and planning. Additional Duties In addition to the above job responsibilities, other duties may be assigned. Minimum Requirements Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire. Experience: 1 year of relevant experience. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. Certification: American Heart Association (AHA) Health Care Provider BLS certification required. Physical Demands Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly. Ability to lift/push/pull 20‑50 lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
