Domestic Violence and Shelters Care Coordinator

Be the bridge to safety and stability for those in need. As a Domestic Violence and Shelter Programs Care Coordinator you will work directly to provide service coordination for individuals who are homeless or survivors of domestic violence. In this role, you will ensure individuals receive safe, therapeutic, and empowering support as they navigate critical life transitions.

Responsibilities and Duties:

Duties include but are not limited to:

  • Completing comprehensive assessments and collaborating with individuals served to develop a care plan that is tailored to their unique needs and goals.
  • Facilitating individual and group support that promotes growth, safety, and empowerment.
  • Maintaining accurate and timely documentation of services provided.
  • Participating as a member of the interdisciplinary team to ensure comprehensive coordination of care.

Crosspoint Human Services offers comprehensive on the job 40 hour domestic violence training. .

Job Requirements

  • Bachelor's degree in social work, counseling, human services, psychology, or related field.
  • Two years work experience in the human services field is preferred.
  • Excellent organizational and time management skills.
  • Valid driver's license and proof of automobile insurance
