Bilingual Case Manager — Housing Stability & Support

Bond Health Staffing in Brooklyn is seeking a Case Manager for a 105-unit Permanent Supportive Housing program. The role requires frequent contact with residents and an emphasis on Housing First approaches, using evidence-based practices in the office and at residents' homes. You will maintain a high-risk caseload, conduct assessments, develop person-centered plans every 6 months, link residents to benefits, coordinate with the psychiatrist, and support community integration. #J-18808-Ljbffr Bond Health
