Senior Social Worker - Dialysis Care & Patient Support
Fresenius Medical Care
Fresenius Medical Care provides psychosocial services to dialysis patients in-center and at home, collaborating with the interdisciplinary team to support rehabilitation and quality of life.
This role assesses psychosocial status, develops care plans, offers counseling, educates patients and families about ESRD, transplant options, and connects with community resources to maximize adherence and outcomes.
