Description Home Health Physical Therapist Opportunity! Monday-Friday, 8a-4:30p Full Time or Part Time Family Home Health is seeking a Physical Therapist who is ready to make a difference in the lives of older adults. Essential Functions include but are not limited to:

- Participate in the development of the total plan of care.
- Maintain appropriate clinical records, clinical notes, and report to the physician any changes in the patient's condition. Submit these records in accordance with the organization policy
- Maintain contact / communication with other personnel involved in the patient's care to promote coordinated, efficient care
