From nurses, therapists, and home health aides to scheduling coordinators, customer service specialists, administrative staff, and leadership—every person at HCR plays an important role in supporting our patients and one another.What You'll DoYou will evaluate each patient's physical abilities and functional limitations through comprehensive assessments and diagnostic tests.You will develop personalized plans of care that target functional improvement, safety, and independence—modifying as needed in consultation with the physician.You will deliver hands-on therapy in the home, using evidence-based techniques to help patients meet their mobility and strength goals.You will educate patients and caregivers on therapeutic exercises, safe mobility, and the use of prosthetics, orthotics, and adaptive equipment.You will document progress accurately and promptly in the patient's clinical record, maintaining clear communication with physicians and the interdisciplinary team.You will ensure a safe environment for patients by practicing proper equipment handling, adhering to safety protocols, and collaborating with other care providers.What You BringYou are a graduate of an accredited Physical Therapy program; DPT preferred.You hold a current, valid Physical Therapy license issued by the New York State Department of Education.You have at least one year of post-licensing, practical experience as a licensed physical therapist.You are a clear communicator, with excellent critical thinking, and problem-solving skills.You have a passion for helping others regain confidence and independence through compassionate care.Why Choose HCR Home CareA respected, mission-driven organization with a People-First culture.Competitive pay and full benefits package.Mileage reimbursement and flexible scheduling.Opportunities for ongoing training, professional development, and advancement.Supportive leadership that values your expertise and empowers clinical excellence.Work EnvironmentHome-based role providing care across assigned territories.Classified as medium work (occasional lifting up to 50 lbs).Involves regular standing, walking, and interaction with patients in their homes.Your expertise helps people move forward—literally.
