Description

• Conduct patient assessments in both inpatient and outpatient settings, crafting personalized treatment plans that promote healing
• Implement physical therapy interventions aimed at enhancing mobility, building strength, improving balance, and restoring function
• Work closely with a multidisciplinary team which includes physicians, nurses, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families on therapeutic objectives, discharge processes, and home exercise regimens
• Maintain comprehensive documentation encompassing evaluations, treatments, and progress in line with facility protocols and regulatory mandates
• Engage in collaborative care rounds and interdisciplinary meetings to promote cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• Certification in CPR is preferred but not mandated
• Additional specialized certifications (i.e., neurological, geriatric) are advantageous but not essential

Skills

• Prior experience in a related setting is highly preferred
• Expertise in managing patients with complex medical histories and mobility challenges
• Excellent teamwork and communication abilities
• Proficiency with digital charting systems and clinical documentation
