PRN HOSPICE MEDICAL SOCIAL WORKER (MSW)A Path of Care Hospice in Coalgate, OK is seeking a PRN Medical Social Worker (MSW, LCSW) to support patients and families with compassionate, skilled social work services. Responsibilities include:Assessing social and emotional factors affecting patient healthParticipating in the development of individualized plans of careMaintaining accurate clinical and progress notesIdentifying and coordinating community resources to support patients and familiesAssisting with discharge planningAttending in‑service trainings as neededConsulting with interdisciplinary team members to ensure well‑rounded patient carePracticing infection control and following universal precautionsRecognizing and reporting any signs of abuse in accordance with policy and lawAssisting with office tasks and new‑staff orientation when neededYour work ensures patients receive holistic, compassionate care that supports both their medical and emotional needs.QualificationsMaster 's degree in social work from a CSWE‑accredited programAt least 1 year of social work experience in a healthcare settingCPR certification or ability to obtain within 60 days of hireLCSW preferred, though multiple factors will be consideredStrong verbal and written communication skillsExcellent organization, time management, and problem‑solving abilitiesAbility to remain calm and professional in stressful situationsFriendly, compassionate demeanor and ability to work well independently and as part of a teamWork SchedulePRNFlexible scheduleReady to Join Our Team?The application process is quick and easy.
