Shift: Monday through Friday, (08:00 AM - 04:30 PM)

Position Overview

We are seeking a dedicated and experienced Travel Registered Nurse (RN) for a contract opportunity with Client supporting our Clinical Care Services segment. In this role, you will provide high-quality home health care, serving as a case manager for a dedicated group of patients. You will work independently in the field while collaborating closely with an interdisciplinary clinical team to ensure optimal patient outcomes.

• Location: Fully On-Site (Field-based coverage in Connecticut)
• Coverage Areas: Harwinton, Burlington, Thomaston, Plymouth, Bristol, Wolcott, and Torrington
• On-Call/Weekend Requirement: None
• Travel/Mileage: Mileage will be fully reimbursed for travel between patient visits (from the first patient of the day to the last).

Key Responsibilities:

• Case Management: Act as the primary case manager for an assigned panel of home health patients, coordinating all aspects of their clinical care.
• Patient Assessments: Complete comprehensive clinical assessments, including patient admissions, re-certifications, and discharges as required.
• Care Collaboration: Partner and communicate effectively with the broader interdisciplinary healthcare team, physicians, and family members to execute and modify care plans.
• Documentation: Ensure accurate, timely, and compliant clinical documentation regarding patient status and interventions.

Qualification and Requirements:

• Education: Associate Degree in Nursing (ASN) or higher.
• Experience: Proven, hands-on Home Health experience is required.
• Certification: Active Basic Life Support (BLS) certification.
• Screenings: Must be willing to complete and pass required occupational health screenings.

Preferred Skills and Attributes:

• Proficiency and prior experience with MatrixCare and Homecare HomeBase (HCHB) electronic medical record (EMR) systems.
• Strong self-management, communication, and critical thinking skills suited for autonomous field work.
