Veterans Home of California - Fresno

Opened in October 2013, the VHC-Fresno has 300 beds and presently offers Residential Care Facility for the Elderly (RCFE), also known as assisted living. As a new service, the Home is offering Skilled Nursing. The two levels of care provide continuity in the lives of residents in a homelike atmosphere of dignity and respect. All healthcare needs, meals, transportation and activities are provided by the Veterans Home of California Fresno. Skilled nursing care is provided to residents on a 24-hour basis by licensed nurses and certified nursing assistants. Skilled nursing residents have greater access to rehabilitation therapies, nursing care, pharmacy management, structured activities, and clinical dietary services. A memory care program with this level of care provides a supervised environment for veterans with symptoms of confusion, memory loss, difficulty making decisions, solving problems or participating in conversations. The Veterans Home provides California veterans with a living environment that protects their dignity and contributes to their feeling of self-reliance and self-worth. The nearby City of Fresno offers many cultural and educational opportunities for Home residents to enjoy.

Responsibilities

• Provide care and tend to physical needs of the patients according to the patient care plan.
• Perform and document rounds of patient receiving care and report changes in patient condition to the Nurse.
• Obtain specimens for diagnostic testing.
• Complete all required documentation of patient care, provide written and/or oral reports.
• Clean, maintain, and check equipment.
• Make rounds as requested and immediately report unusual resident conditions to physician or the charge nurse. Must adhere to the policies and procedures of the facility.
• Be aware of resident's rights, identifying the potential for resident abuse, report promptly to the physicians or nursing supervisor about any adverse behavior, resident abuse, or suspected abuse by staff.
• Float to other units as necessary.
• Answer call-lights promptly.
• Assure that all residents have monthly/weekly weight taken and recorded; report significant changes to the physician and registered dietitian immediately.

Requirements

• Experience: Shall have a minimum of three months within the last three years.
• Professional license in California.
• BLS from a recognized program (AHA or Red Cross).
• COVID Vaccination: Needs to be fully vaccinated including booster, if eligible, or have a medical or religious exemption*. Medical exemption: no information needed but exemption does need to state a timeframe or if indefinite, needs to state as such. If not fully vaccinated, required to get tested twice a week.
