• Client utilizes their own internal EMR system
• Responsibilities include:
  • Supporting highly complex patients navigating the healthcare system
  • Providing one-on-one patient care and education
  • Developing dedicated care plans
  • Coordinating care with physicians, families, dialysis centers, and integrated care teams
  • Performing Comprehensive Health Assessments and Health Risk Assessments
  • Managing longitudinal care for CKD and ESKD patient populations
  • Medication management and post-discharge follow-up
  • Utilization management and readmission prevention
  • Accurate coding and documentation with HCC experience
  • Administering tests and screenings
• Position includes:
  • In-clinic care at dialysis centers and nephrology practices
  • In-home visits within designated service areas
  • Occasional telehealth appointments
  • Regional flexibility to provide coverage as needed

Requirements

• Current California Nurse Practitioner license
• Board Certified NP in good standing
• ANCC or AANP certification required
• Minimum 2 years of NP experience in:
  • Primary Care
  • Family Medicine
  • Internal Medicine
  • Nephrology
  • Gerontology
  • Palliative Care
• Home care experience preferred
• Working knowledge of chronic care management and medication management
• HCC experience required
• Current DEA licensure/prescriptive authority
• Active CPR/BLS certification
• Active California driver's license required
