As a Home Health Registered Nurse, youll provide vital skilled nursing services to individuals in their homes, ensuring care delivery is compassionate, clinically sound, and personalized to each patients needs. In this contract role, your ability to work independently while fostering strong communication with patients and interdisciplinary teams will be essential for success.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and create individualized care plans.
• Deliver comprehensive nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and document patient progress, promptly addressing changes in condition and communicating with physicians and the care team.
• Educate patients and families about diagnoses, treatment regimens, medications, and overall health management.
• Maintain meticulous documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for integrated patient support.
• Participate in case conferences and ongoing professional development opportunities.
• Ensure compliance with regulatory and agency standards.

Qualifications:

• Active RN license, in good standing
• Minimum one year of recent field-based Home Health RN experience
• Proficiency in Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and insurance
• Strong assessment, time management, and communication skills
• BSN and experience managing high-point caseloads preferred
• Knowledge of Medicare and home health compliance is a plus

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
