AVIR at Del Sol is seeking compassionate and motivated Physical Therapists (PTs) to join our in-house subacute rehabilitation team . We are hiring for Full-Time and Per Diem opportunities. If you're passionate about helping patients regain mobility, strength, balance, and independence, this is an excellent opportunity to make a meaningful difference in the lives of residents while working with a collaborative interdisciplinary rehabilitation team. Responsibilities Evaluate residents and develop individualized physical therapy treatment plans. Provide skilled physical therapy interventions to improve mobility, balance, strength, endurance, and functional independence. Monitor resident progress and modify treatment plans as clinically appropriate. Supervise and collaborate with Physical Therapist Assistants in accordance with applicable regulations. Educate residents and caregivers on exercises, mobility techniques, and safety strategies. Accurately document evaluations, treatment sessions, progress notes, and discharge summaries. Communicate resident progress and clinical recommendations with the interdisciplinary care team. Collaborate with physicians, nursing staff, and rehabilitation team members to provide coordinated, high-quality care. Participate in interdisciplinary team meetings and discharge planning. Qualifications Graduate of an accredited Physical Therapy program. Current Texas Physical Therapist license in good standing. Experience in skilled nursing, subacute rehabilitation, or geriatrics is preferred but not required. Strong clinical, communication, organization, and documentation skills.
